P4 Initiative

The P4 Residency Demonstration Initiative is designed to inspire and examine substantial innovation in family medicine residency training. Sponsored by the American Board of Family Medicine (ABFM) and the Association of Family Medicine Residency Directors (AFMRD), in cooperation with TransforMED, an affiliate of the AAFP, P4 uses a scientific approach to study and measure innovations in search of the most effective ways to educate the country’s future family physicians.

More than 80 residency programs proposed innovations and ideas that would be expected to seed the next set of revisions in the education and training of family physicians.  While many family medicine residency programs are taking new and progressive approaches to physician training, the 14 residencies participating in the P4 initiative will highlight innovations taking place in residencies across the country.

A sampling of innovations from P4 residencies:

  • Enhancement in chronic disease care, patient centeredness and health behavior change. 
  • Increased training in patient-centered medical homes, community practices and other non-traditional training settings. 
  • Expanded training time with enhanced curriculum resulting in advanced degrees and added competencies. 
  • Enhanced practice management, leadership, process improvement and change management training.
